2. Caloric intake/output.

Most significantly, #2. And it doesn't matter one iota if those calories are fat, protein, sugar, or starch. You'll get just as fat off 2000 calories of steamed rice as you will off 2000 calories of Snickers bar batter-fried in pork fat.
